SSM 203–09 Table

The student requests the grade of “incomplete” (“I”) from the instructor. If the request is approved, the instructor gives the student a Request for Grade of Incomplete form (if no grade has been reported) or submits a grade change on the Faculty Center (if a grade has been reported). The student and the instructor complete the form, if applicable.

The instructor:

  1. gives the student copy of the form to the student
  2. retains a copy of the form
  3. forwards the department chair copy of the form to the department

    Note: This information will enable the department to verify the completion of the Request for Grade of Incomplete if the instructor leaves the university within the year of course completion.

    and

  4. assigns a grade of “I” using the Faculty Center if no grade has been assigned yet.

When the course work is completed (whether the student passed or failed), the instructor submits a grade change on the Faculty Center.

The department chair verifies that the grade change has been submitted within the time limit. If the chair approves the request, he or she approves the grade change on the Faculty Center.

If the dean’s office approves, the grade change is approved in the Faculty Center.

The instructor verifies that the grade change was processed accurately.
