EHS 116 Table

The ASU employee (of an academic unit) submits, to the head of his or her academic unit, a request that provides the following information:

  1. the name, gender, and age of the minor student
  2. the intended status (employee or student enrolled in a class or sponsored program associated with the university) with respect to the proposed experience
  3. description of the educational intent

    and

  4. the tasks to be performed.

The academic unit head, or designee:

  1. reviews, then approves or denies the request, for each minor, in writing

    and

  2. if approved, forwards a copy of the request to EH&S with a statement that the proposed activity represents an approved program.

Environmental Health & Safety and Risk Management reviews, then approves or denies the request, for each minor, in writing, and if approved:

  1. ensures that appropriate inspections are performed of the laboratory or area where hazardous materials and processes are employed

    and

  2. alerts the requesting employee to any measures that must be taken to assure the safety of minors in the laboratory or area where hazardous materials or processes are employed.
If denied, EH&S notifies the requestor of the reason(s) for the denial.

The academic unit head, or designee:

  1. confirms receipt of the responses from EH&S for each request
  2. ensures compliance with all training and safety measures identified by EH&S associated with the approval
  3. requests and obtains the parent or guardian consent form(s), from the parent or guardian of each minor student
    Note: The forms must be signed by the parent or guardian and received by the academic unit before the minor is permitted into the laboratory or area where hazardous materials or processes are employed.

    and

  4. retains the signed parent or guardian consent forms for a period of five years.
