SSM 305–07 Table

The employee notifies the supervisor of injury and, if necessary, seeks medical attention immediately.

The employee or the supervisor contacts the State of Arizona Workers’ Compensation Early Claims Reporting Service at 602-542-WORK or 1-800-837-8583 within the first 48 hours. This number is available 24 hours/7 days a week.

The supervisor:

1. completes the Employer’s Report of Industrial Injury and Supervisor’s Accident Investigation Report
2. has the injured employee sign and date the State Risk Management Authorization form

   and

3. forwards the completed forms to Human Resources/Customer Service Center at the Tempe campus within five days from the date of injury.

Human Resources contacts the employee or supervisor for additional information if necessary.
