SSM 202–07: Leave of Absence for Undergraduate Students

Purpose

To allow an undergraduate student to take a Leave of Absence (LOA) from the university for one or two consecutive semesters

Applicability

[horizontal rule]

This policy applies to undergraduate students who:

  1. are degree-seeking
  2. are registered during the semester immediately prior to the beginning of the LOA
  3. are in academic good standing, on academic warning, on probation, or on continuing probation
  4. are not the subject of any hold that would prevent registration

    and

  5. have submitted all outstanding high school and/or transfer transcripts.

Policy

To apply for a Leave of Absence (LOA), an undergraduate student must:

  1. complete the Leave of Absence Request
  2. obtain the signature of the college (if required)
  3. obtain the signature of the International Student Office (if the student is in the U.S. on an F1 or J1 visa)

    and

  4. submit the form to University Registrar Services.

  2. If LOA request is for two semesters or less and student is not in the U.S. on an F1 or J1 visa, skip to step 7.
  3. Request signature of authorized individual in the International Student Office (if in the U.S. on an F1 or J1 visa).
International Student Office
  1. Review LOA request.
If the student meets requirements:
  1. Sign LOA and return to student.
If the student does not meet requirements:
  1. Do not sign LOA request and notify student of ineligibility.
College
  1. Review LOA request.
If the student meets requirements:
  1. Sign form and return to student.
If the student does not meet requirements:
  1. Do not sign LOA request and notify student of ineligibility.
Student
  1. Return completed application to University Registrar Services.
University Registrar Services
  1. Verify that the LOA request is complete.
  2. Verify student meets LOA requirements
If the student does not meet requirements:
  1. Notify the student in writing that LOA has been disapproved.
If the student meets requirements:
  1. Process LOA in OASIS
  2. Notify the student and student’s college in writing that LOA has been approved.
University Registrar Services
  1. Prior to the student’s return to ASU, notify the student in writing of LOA expiration.
Student
If additional leave is necessary:
  1. Submit a new LOA request, following the procedures in this policy. If the total requested LOA duration is greater than a year, the additional time will not be granted without college approval.
Readmitted student
If additional leave is not necessary:
  1. Complete any outstanding items (e.g., submit transcripts from institution attended during LOA, meet with advisor, complete any to-do items, and clear any holds).
