Purpose

To provide up to $2,500 in financial assistance to support eligible staff members who receive fertility treatment.

Applicability
Benefits-eligible, regular classified staff, university staff, and non-faculty administrators.

- Eligible employees may receive the fertility benefits subsidy when the following conditions are met:
- They receive medical services or prescriptions as part of fertility treatment.
- They are benefits eligible both at the time they receive services and submit a subsidy request.
- The service or prescription is not covered under the medical plan.
- The service or prescription is received on or after July 1, 2019.
- The subsidy request is submitted within six months after the date the service or prescription is received.
- The employee has not previously received the maximum fertility benefits subsidy.
- If both potential parents or domestic partners are ASU employees, only one fertility benefits subsidy is allowed per family.
- The fertility benefits subsidy amount is determined by the university and is subject to change.
- The fertility benefits subsidy is available to employees in a pay status immediately upon hire or benefits eligibility.
