Classroom Exam Scoring: Processing Policies and Procedures

Before Giving the Exam
  1. Exams must be completed on Scantron (formerly NCS) ExamSYSTEM II  form # 229633 [200KB, PDF] answer documents (bubble sheets),available for purchase through the ASU Stores Catalog.
  2. Currently, multiple answers for a single question cannot be processed (only one answer for each question can be correct).  Universal credit (where all answers are correct) may be given for an item by marking more than one or all responses on the key.  You may omit the question entirely by leaving all responses empty on the key for that question.
Prepare Exams for Drop Off
  1. A left justified identification (ID) number is REQUIRED on ALL answer sheets.  It must be from 2 to 10 digits long.  ID must be a unique number.  The 10 digit ASU ID, the 7 digit Posting ID, or a pre-set private number are some examples of what could be used.  Students' name fields must also be left justified.
  2. A #2 pencil must be used to make heavy black marks.  Answer sheets completed with pens cannot be processed.  If bubbles were filled with pen, mark over them with a #2 pencil.  Erase stray marks or writing anywhere but the place indicated.  Marks placed near the black horizontal bars or the black squares renders scoring impossible.  Forms should not be bent, folded, or otherwise mutilated.  Changed responses should be erased completely with no visible smudges.
  3. Arrange answer documents so that they are all facing in the same direction.  Improperly sorted forms will interrupt the optical scanning machine and delay processing considerably. 
  4. Submit only completed sheets for processing.  Make sure that no other papers or blank answer sheets are included in the envelope, as they interrupt the scanning machine and delay processing.
  5. Complete an answer key on an ExamSYSTEM II form with a #2 pencil.  Label this sheet, "KEY" in the NAME grid.  Leave the ID grid BLANK on the key sheet.  Place answer key on top of bubble sheets.
  6. Place answer sheets in a campus envelope (one envelope per answer key).  Do not send tests through campus mail!   Do not use staples, paper clips, labels, or rubber bands on forms as these can mutilate forms and result in delayed processing and/or improper scoring.

  3. Complete one purple Examination Scoring Request Form [150KB, PDF] per envelope (one envelope per version of exam).  Be sure to specify CLASS NUMBER, and test name/number/version. 
  4. When you drop off exams, please time stamp the exam request form by inserting the upper right corner of the purple form into the time-stamp machine.  Attach the purple form to the outside of each envelope with a single piece of tape.
  5. OR

    Exam answer sheets that need to be scanned and processed by University Testing and Scanning Services may now be placed in centrally located courier drop boxes for delivery by ASU Mail Service couriers. The couriers will make two trips daily, morning and afternoon, from each drop box to UTSS. YOU MUST PRINT A COURIER REQUEST FORM from: http://sclapp.asu.edu/Enterprise/ Drop box locations are:

Convenient Online Access to Exam Results!!!!
  1. Work requests are processed within one business day.  When the work is completed, the results will be uploaded to your MyClasses portion of MyASU.  Depending on the reports you request, you will have access to a PDF file and/or an XLS file. You can find detailed information on this feature and how to download your files at:  http://help.asu.edu/node/1274.
  2. Different versions of an exam CANNOT be merged for statistical .PDF reports
  3. We CAN merge multiple versions into one Excel file. This file will contain ID #, Name & Raw score.  (Please check “Merged Files” on the Exam Scoring Request form for this option.)

Pick up
  1. You will be contacted at the phone number or email address specified on the purple examination request form, unless you instruct otherwise. 
  2. You must show ID when picking up your scanned exam bubble sheets.  They can be picked up after one business day (or if you see results sooner on MyASU) OR for convenience, you may pick up the exam bubble sheets when submitting your next request.  Exams must be picked up within 30 days.  UTSS cannot store or destroy these documents. 

Please Note:

We process a high volume of scanning and do so on a “First In, First Out” policy.  Every effort will be made to process your exams as expeditiously as possible.   If a special situation exists that requires your exams to be processed sooner than one business day, please talk with a scanning staff member when you drop off. 

In order to prevent interrupted scanning and exam processing delays, please do not call within the 24 hour processing window to see if your exams are finished.  Exams are completed if you see your reports are available on MyASU.
