How to Use the NCS System

Class exams must be completed on NCS
examSYSTEM II form #229633 available for purchase through the ASU Stores Catolog.
A #2 pencil must be used to make heavy black marks. Answer sheets completed with pens or markers
result in improper scoring and cannot be processed. Be sure answer sheets have no stray marks or writing
anywhere except where indicated (on either front, back or sides). Any marks placed near or in the areas of
the black horizontal marks or black square renders scoring impossible. Forms should not be bent, folded or
otherwise mutilated. Changed responses should be erased completely with no apparent smudges.
Students' names and ID numbers must be left justified. That is, the first character
must appear in the leftmost column of the grid. An identification number is required on ALL answer
sheets. It must be left justified and from two to ten digits long. Do not use duplicate ID numbers
.
Complete an answer key on examSYSTEM II form #229633 with a #2 pencil. Label this sheet "KEY" in the
NAME grid. Leave the ID grid blank on the key sheet.
Currently, multiple answers cannot be processed and different forms of the
test cannot be merged for statistical paper reports. We can merge the alphabetical lists of
multiple versions and generate a merged listing of the last four digits that were bubbled in the ID
section and, if applicable, the first three digits in the special codes section. Please check "merged
printouts" on the scoring request form for this. We can also merge multiple versions into one disk file.
This file will contain ID number, name and raw score. Please check "merge files" on the scoring request
form for this option and submit a blank 3 1/2" formatted IBM disk.
Universal credit may be given for an item by marking all responses.
Arrange answer sheets so that they are facing the same direction. Improperly sorted forms will
interrupt the optical scanning machine and delay processing considerably. Place answer key on top.
Submit only completed answer sheets for processing. Make sure that no other papers or blank answer
sheets are included as they interrupt the scanning machine and delay processing.
Place answer sheets in a campus envelope (one envelope per answer key). Do not send test
through campus mail. Do not use staples, paper clips or rubber bands to group forms as these can
mutilate forms and result in delayed processing and/or improper scoring.
Bring the forms to UTS. UTS does not provide delivery or pickup services. The
client is solely responsible for dropping off and picking up his/her exams.

Complete one purple EXAMINATION SCORING REQUEST
form (pdf format) per envelope. The forms are also available by
contacting our office at 5-7146 or at uts@asu.edu. If
you download the form, please print it on purple paper. Be sure to specify
the instructor's name, six to eight character course name/number, test
name and number, number of questions to be
processed, local phone number for pickup and types
of reports desired. It is extremely important to indicate the number of
tests that you have submitted for processing. (NOTE: It is to your advantage
to check student names off a roster as they turn in their tests so that
you will have a precise record of who has actually taken the test.) Insert
the upper right corner of the purple form into the time-stamp and then attach
the purple form to the outside of the envelope with tape.
Work requests are processed within 24 hours. Your office will be contacted at the local number
specified on the EXAMINATION SCORING REQUEST form when the work is completed. If you are
not available, a message will be left at that number. To pick up exams and score reports, the instructor
or runner must present photo-bearing ID and print his/her name as recipient of the
materials. Test results are on disk at UTS for one week only. UTS does not share or make public
any test data.
