CLEP Test Registration Form

The registration form below cannot be transmitted electronically.
You may complete the form on the computer, print it, and then mail it or bring it to UTS.
WE CANNOT ACCEPT PAYMENT BY CASH, CREDIT OR DEBIT CARDS.

REGISTRATION DEADLINE: 4:30 PM ONE WORKING DAY before the test. Walk-ins are not allowed.
REGISTRATION FEE: $25.00 PERSONAL CHECK OR MONEY ORDER ONLY made payable to Arizona State University.
CLEP TEST FEE: On the day of the test, $70.00 is due and payable to CLEP by personal check, money order, MasterCard, VISA, Discover, American Express, or JCB. (Sorry, we cannot accept cash for either fee.)

If you are registering by mail, we will phone you to confirm your scheduled test date. If you do not receive your admission ticket prior to your test date, it is your responsibility to contact this office. Our phone number is 480-965-7146.

SPECIAL ACCOMMODATIONS: To receive disability accommodations you must be registered with the ASU Disability Resource Center and be approved for special accommodations. You will also need to arrange an individual test appointment with our Program Coordinator. Please do not sign up for a group testing time if you plan to request special accommodations.

Will you need an optional essay? Yes No (NOTE: If you need an Optional Essay, you must request it and pay an additional $10 fee at least two weeks in advance. You may take the multiple choice portion of the test separately from the essay. English Composition with Essay does not require an extra fee or advance request. However, you will not receive an immediate score report after you take that test—as CLEP must first evaluate your essay. Your score report will arrive about 2 to 3 weeks later.)

Method of Payment for the CLEP Fee: This payment will be processed when you take the test. When you arrive for the test, you will need to present a check or money order, or enter into the computer the credit card number, expiration date, and name as it appears on the credit card.

CANCELLATION & TRANSFER POLICY : The test registration fee is nontransferable and nonrefundable.

RESTRICTIONS: You will not receive credit on a CLEP test for a lower-level or prerequisite course when you have already received credit at that level or at a higher-level within the same field. If you have accumulated more than 100 credit hours, it is possible that you will not receive CLEP credit. Check with your advisor and the department in which you are majoring before you register. You must wait six months to retake the same CLEP test.

IF YOU ARE TAKING ENGLISH COMPOSITION WITH ESSAY or FRESHMAN COLLEGE COMPOSITION: ASU will neither honor CLEP scores to fulfill the “First-Year Composition” requirement nor award elective credit for these tests. However, CLEP tests in this area may qualify you to register for ENG 105. Alternatively, you may take the English Placement Test to qualify for ENG 105 provided you obtain an acceptable score. There is no charge for the English Placement test. However, you may take it only once.

STUDENT AGREEMENT

I agree to the policies and conditions of this test as written on the registration form and attached information. I understand that, if I violate any rules or directions related to the administration of this test, the following actions may occur: I will be dismissed from the test session, my test score will be cancelled, I will forfeit my testing fee, and a written report will be filed by University Testing Services with the test company regarding the incident.
