Personal Wellness Profile
The Wellsource® Personal Wellness Profile™ provides a thorough wellness-based health assessment where participants can learn about their current health status, receive personalized wellness recommendations, and learn about ASU Wellness Resources. Participants will complete an online health questionnaire and then receive the online Personal Wellness Profile™ Personal Report with Smart Exits Links to ASU Wellness programs and services.
