Program Components
Personal Wellness Profile™ Health Assessment
The Wellsource ® Personal Wellness Profile™ Health Assessment provides a thorough wellness-based health assessment where participants can learn about their current health status, receive personalized wellness recommendations, and learn about ASU Wellness Resources. Participants will complete an online health questionnaire and then receive the online Personal Wellness Profile™ Personal Report with Smart Exits Links to ASU Wellness programs and services.

The Wellsource Online Wellness Center is a comprehensive online health improvement and wellness education website where participants can learn how to improve their health, prevent disease, manage chronic health problems, and obtain reliable instruction for improving quality of life.
