Medical RecordsASU Campus Health Service medical records are kept confidential and are only released by patient request or in those circumstances designated by state or federal regulations. Inactive medical records are kept onsite for 2 years after the last patient encounter and at offsite storage for 8 years. If you require copies of your ASU Campus Health Service medical record you will need to fill out the Authorization form. Copying fees may apply. For those students with ongoing medical conditions or special medical circumstances, it is important for the Campus Health Service treating clinician to obtain copies of your past medical records. As a courtesy to our patients, by filling out the Authorization form, we will request copies of those records. Arizona law requires parental consent to provide medical treatment for individuals under the age of 18. If you are under the age of 18, a Parental Consent form must be filled out by your parent or guardian.
