Like most health care providers, the Campus Health Service charges for office visits and for the other medical services we provide - lab tests, immunizations, x-rays, EKGs, acupuncture, nutritional consulting, counseling*, minor surgery, medical supplies and medical equipment. We accept all health insurance plans and will bill your health insurance company directly for all services.You are responsible for the charges not covered by your insurance plan. *Counseling services are provided by psychologists from Counseling and Consultation. Q. Can I receive health services if I do not have insurance? A. Absolutely. The Campus Health Services provides services to all ASU students regardless of their health insurance status. Q. Will my insurance pay for all charges incurred at the Campus Health Service? A. Many ASU students have health insurance that will cover the costs of medical services (including office visits and ancillary services, such as lab tests and x-rays) provided in the Campus Health Service. The Campus Health Service accepts all health insurance plans and will bill your health insurance company for the services you receive as an out-of-network provider. You are responsible for the charges not covered by your insurance plan. Q. What insurance companies are Campus Health contracted with? A. Aetna, Arizona Foundation,Blue Cross/Blue Shield, CIGNA, FirstSource, Great-West, Health Net, Private Health Care Systems Inc, Schaller-Anderson, United Health Care Q. How do I bill my private insurance company? A.The Campus Health Service will bill your plan for you. Q. Do I have to pay a co-payment? A. Yes, we will bill your student account the co-pay dictated by your insurance carrier. Q: What if I do not want to bill my private insurance because of sensitivity issues? A: Please let your Campus Health Service providers know during your visit and they will make arrangements for you to pay for the charges incurred. Q: Does the Campus Health Service pharmacy accept my insurance card? A: No. We currently are not able to bill your insurance for pharmacy charges. However, our pharmacy charges are quite reasonable and can be billed directly to your Student Account. Q. How do I pay my bill? A. You can pay your Campus Health Service charges on the day of service by Cash, check, MasterCard, Visa, Suncard. If you do not pay on the day of your visit your charges will be transferred to your ASU Student Account. Q. My Campus Health Service charges are wrong, how do I fix them? A. All appeals must be done in writing. E-Mail the Student Business Services office: stubusserv@asu.edu. Or mail a letter to Student Business Services, PO Box 870303, Tempe, AZ 85287-0303. Include your ASU ID number and an explanation as to why your late fees should be reversed. ContraceptivesQ. Do I need a pelvic exam if I want to get birth control pills? A. No, you do not need a pelvic exam the day you get a prescription for birth control pills. However, when you are sexually active we recommend a Pap Smear at least once a year . We also recommend testing for Chlamydia, HIV, syphilis and depending on symptoms for gonorrhea if you change your sexual partner within that year. So a pelvic exam should be performed within one year of starting on birth control. Q. What about the "Morning After" pills? A. We dispense Plan B which is most effective if taken within 72 hours but yet offers some pregnancy protection up to 5 days after unprotected intercourse. Therefore, please call (480) 965-3338 for more information. Q. Do you have condoms? A. Yes, you can pick up three free condoms each day that you visit the Health Shop. Q. Do you have an ophthalmologist at the Campus Health Service? A. We have 5 Internists, 3 Emergency Physicians, 7 Family Practice physicians, 2 GYN Nurse Practitioners, 3 Family Nurse Practitioners, 1 Psychiatric Nurse Practitioner and 3 SportsMedicine specialists on staff. Medical RecordsQ. How do I get a copy of my medical records?A. It is a simple matter of filling out the appropriate Authorization to Release Medical Information forms. These are located in the Main lobby at the scheduling desks or by clicking on the link. In order to process your request in a timely manner, you must provide your name, ASU ID#, address, and phone number. In addition, if you wish for your records to be released to another party (i.e. an outside clinician, attorney, insurance company, etc.) their complete address and phone number is required. You also have the option of requesting your entire medical chart or specific information from your chart. Depending upon the number of pages copied and where the record is being sent, there may or may not be a fee. Q. How do I get a copy of my medical records from my outside doctor sent here to Campus Health Service? A. This is accomplished by filling out the appropriate Authorization for Release of Medical Information form. This form is found in the Main Lobby at the scheduling desks or by clicking on the link. In order to process your request in a timely manner, the following information is required: The name of the hospital or doctor from where the records are being requested, a complete address and/or fax number, and a phone number. In addition, your name, address, and ASU ID# are also required. As a courtesy, this form can be mailed or faxed by our Medical Records staff at no cost. Q. My child is under 18 years of age. Can he or she be seen at Campus Health Service? A. In order for any student under the years of 18 to be seen at the clinic, Arizona law requires parental consent for medical, surgical, and psychiatric treatment to be delivered. Therefore, a Parental Consent for Medical Care For Underage ASU Studentsform must be filled out by the parent or legal guardian. Food SafetyQ. When do I need to fill out the Food Waiver form? A. The Food Waiver form must be filled out for any on campus event involving food. The only exception is when you use the on-campus approved caterers i.e. ARAMARK; Atlasta; Chartwells or Sodexho Sports & Leisure Q. Can I sell or give away food I make at home? A. NO. All food and beverage sold or given away to the public, must be prepared or purchased at a commercial food establishment with a valid health permit. Q. How do I get started if I want to organize an event involving food?
Q. When do I need to fill out the Temporary Food Application? A. The Temporary Food Application must be filled out when your group hires an off campus food vendor for your event which is open to the general population by invitation or advertisement. Q. Who is responsible for paying the permit Fee for a Temporary Food Permit? A. Who pays the Fee is a contractual matter between you and the Food vendor. The Fee is due when the application is submitted at least 14 days prior to the event. |
