Fee InformationEligible students may opt to have fees charged to their student accounts. INSURANCE BILLING- The Campus Health Service currently bills all health insurance plans and encourages all students to maintain adequate healthcare coverage. The Campus Health Service physicians and nurse practitioners are currently in the process of becoming a preferred provider (i.e., "in network") on many of the larger insurance plans. The insurance companies we are currently contracted with are Aetna, Arizona Foundation, Blue Cross/Blue Shield, CIGNA, First Source, Greatwest, Health Net, Humana, PHCS(Private Healthcare Systems, Inc), Schaller-Anderson and United Health Care. The Campus Health Center will attempt to bill claims as an out-of-network provider for all other insurance plans. If an insurance carrier denies payment, the student's ASU Student Account will be billed. Please note that all services not covered by the insurance plan will be billed to the student's ASU Student Account. The Campus Health Service will continue to provide health service to all ASU students regardless of their health insurance status. MEDICAL CARE There is a visit fee for urgent care, primary care, women's health, nurse clinic and specialty consultant visits. MEDICAL PROCEDURES Minor surgical procedures, injections, diagnostic tests, X-rays, and other medical procedures are provided at a reasonable cost to students. MENTAL HEALTH Please visit the Counseling & Consultation web site for fee information. PHARMACY Prescription and non prescription items may be purchased at theCampus Health Service Pharmacy, which is conveniently located on the first floor of Campus Health Service. The Pharmacy participates in the State of Arizona's membership in the Minnesota Multi-state buying group, resulting in lower prices on many often prescribed medications. Pharmacy Price Index NUTRITION Nutrition counseling fees are $10 to $20 per visit. |
