Tempe campus testing procedure
Qualified Student Responsibilites:
1.
a. Continuing ASU students must enroll within 3 days of their priority enrollment appointment.
b. New ASU students should attend an orientation session as early as possible upon admission to the university and enroll in classes.
2. Meet with the assigned DRC Disability Access Consultant (DAC) early in the semester to:

Request testing accommodations |
| b. |
Verify testing accommodations eligibility |
| c. |
Read and sign a DRC Testing Services Contract which details student responsibilities for ensuring receipt of testing accommodations at the DRC. |
| d. |
Provide course syllabi to assist DAC in completing Side One of the Academic Test/Quiz Administration Authorization Form (Gold Sheet) for each class in which testing accommodations are authorized. |
| e. |
Pick up DRC Faculty Letter/s verifying DRC registration and accommodations eligibility. |
3. Notify each faculty member/instructor of the need for testing accommodations.
Note: Students are advised to meet with faculty and set up all necessary testing accommodations at the beginning of each semester.
4. Meet with faculty to:

Request the faculty/instructor review appropriate accommodations on Side 1 of the Academic Test/Quiz Administration Form then complete and sign Side 2 of the form. |
5. Return the completed Academic Test/Quiz Administration Authorization Form to the DRC Supervisor of Testing Accommodations as follows:

At least three working days before exams or quizzes |
| b. |
At least seven working days before exams or quizzes if tests must be converted to an alternative format (e.g., Braille, large print, audio tape, or E-Text) |
| c. |
At least seven working days in advance for evening classes in which DRC readers, scribes, or assistants are needed |
| d. |
At least ten working days before the last day of class for final exams.
Note:Students may fax completed Testing Accommodations forms to 480/965-9409, mail completed Testing Accommodations forms to the DRC Supervisor of Testing Accommodations, Disability Resource Center, Arizona State University, Tempe, AZ 85287-3202, or deliver completed Testing Accommodations forms to Matthews Center, 111. |
| e. |
In the event the faculty/instructor changes/adds a test date, it is the responsibility of the student to submit a Change or Addition of Test Date/Time form (Green Sheet) to the Supervisor of Testing Accommodations at least three working days prior to an exam. |
| f. |
Students may not bring exams the DRC Testing Service. All exams must be sent by the instructor or picked up by a representative of DRC. |
6. Contact the DRC Supervisor of Testing Accommodations to confirm the receipt of faxed or mailed Testing Accommodations forms (480/965-4731).

DRC Disability Access Consultant Responsibilities
1. Meet with each qualified student requesting testing accommodations and determine appropriate accommodations based on documented need.
2. Provide a copy of the Testing Services Contract to the Supervisor of Testing Services.
3. Fill out the Authorization to Administer Academic Test/Quiz form and initial appropriate accommodations with the student, in person or electronically when that option is available.
4. Request that the student take the Academic Test/Quiz Administration Authorization Form to the faculty/instructor for completion and signature and then submit it to the Supervisor of Testing Services at the DRC by the required deadline.
5. Work with the Supervisor of Testing Services and student as needed.

Faculty/Instructor Responsibilities
1. Complete the instructor authorization section of the DRC Academic Test/Quiz Administration Authorization Form (Side 2) provided by qualified students with disabilities:

Include the length of test time allowed the rest of the class if it differs from the full class period. |
| b. |
Work with the DRC Supervisor of Testing Services to establish a time that allows for extended time when the student has back-to-back classes or other extenuating time restraints. |
| c. |
Specify any materials or equipment approved for student use during the test (e.g., calculator, notes, software etc.) |
| d. |
Return the completed Academic Test/Quiz Administration Authorization Form to the student for delivery to the DRC or fax/e-mail the form back to the DRC Supervisor of Testing Accommodations.

2. Fax (480/965-9409) or deliver the exam to the Supervisor of Testing Accommodations the day before each exam.
3. Email or call the Supervisor of Testing Accommodations (480/965-4731) upon receipt of completed exams to request destruction of copy on file at the DRC Testing Office.

Delivered tests must be in a sealed department letterhead envelope marked with the instructor's name, test date, and student's name. |
| b. |
Emailed, delivered or faxed test materials requiring alternative print conversion (e.g., Braille, large print, audio tape, or E-Text) must be received by DRC a minimum of five working days before the scheduled test date. |
| c. |
Follow the same procedure for pop quizzes as for regular tests.
Note:DRC requires additional time to convert test materials and has limited resources for accommodating students and providing for proctors. DRC Testing must be given both notice and the text of the pop quiz three working days in advance of the pop quiz or the instructor will have to administer the quiz. See #5 below: Follow specified procedures when administering tests with accommodations for students with disabilities. |
4. Special test accommodations requirements for qualified students who are blind/visually impaired:
| a. |
Deliver all test materials to the DRC testing accommodations office (Matthews Center 111) a minimum of five working days before the test date so that the DRC can check for any charts, graphs, diagrams, or other visual information that must be converted to an alternative format (e.g., Braille, raised line drawings) in advance. |
| b. |
If the Academic Test/Quiz Administration Authorization Form states that a reader or a scribe is appropriate as an accommodation for an exam(s) in mathematics, foreign language, the sciences, or other technical fields, work with the Supervisor of Testing Accommodations to provide a TA, graduate assistant, or otherwise qualified student to administer the exam. |
5. Follow specified procedures when administering tests with accommodations for students with disabilities:
| a. |
Contact the student's assigned DRC Disability Access Consultant to discuss appropriate accommodations. |
| b. |
Implement the accommodations agreed upon. |
| c. |
Deliver or email (faxes are often difficult to read and scan.) print materials to the DRC a minimum of five working days before the scheduled test date if the test requires alternative print format (e.g., Braille, large print, audio tape)
Note: The DRC will deliver converted materials back to the academic department designee a minimum of one day before each scheduled test. Department designee must sign and date the Testing Accommodations sheet presented by the DRC carrier. |
6. Contact the DRC Supervisor of Testing Services (480/965-4731) if DRC aides (e.g., reader, scribe, proctor) are required for night classes or administration of exams in the classroom.

DRC Supervisor of Testing Accommodations Responsibilities
1. Collect authorization to administer academic test/quiz forms.
2. Send an email to each student receiving testing accommodations with an attachment of their test schedule.
3. Work with the DRC Disability Access Consultants and the DRC Assistive Technology Center before the first scheduled examination, to schedule training time for qualified students on assistive technology.
4. Contact faculty with a test reminder two days before each exam to be administered at the DRC.
5. Notify the student one day before the scheduled exam date if DRC has not received information requiring alternative format conversion.
Note: The Supervisor of Testing Accommodations will work with the student and faculty to reschedule the exam or to identify appropriate alternatives, as appropriate.
6. Send an electronic copy of the DRC Testing Incident Report form to the faculty/instructor, the student, and the Disability Access Consultant specifying all testing changes or problems that arose during the testing process including the actions taken for correction.
7. Ensure that all test materials, including all alternative format materials, are delivered to the designated academic department/office for faculty pick up following each test administered by the DRC.
8. Obtain departmental representative printed name and signature when completed test materials are delivered back to the department.
