Tempe campus notetaking procedure

Qualified Student Responsibilities:

1.
a. Continuing ASU students must enroll within 3 days of their priority enrnollment appointment.

b. New ASU students should attend an orientation session as early as possible upon admission to the university and enroll in classes.

2. Meet with the assigned DRC Disability Access Consultant (DAC) to:

a. Request note-taking services
b. Complete Note-Taking Agreement Contract
c. Clarify in-class note-taking procedures
d.
Pick up two note-taking packets for each class, including letters to faculty verifying note-taking eligibility and the in-class announcement that faculty will read in class to solicit volunteers
e. Request backup procedures to cover notetaker absence or if no one in the class volunteers (e.g., a tape recorder and tape transcription or student hourly notetakers)
f. Request notetaker training and ongoing support as needed.
Note:.

The student with a disability is the manager of the Note-Taking Agreement Contract. The student hires, evaluates, trains, tracks paperwork, and terminates notetakers.

3. Meet faculty prior to first class to discuss the need for note-taking accommodations, explain note-taking procedures, and provide faculty with the note-taking packets.

4. Ask the instructor for assistance in selection of the most qualified notetaker from the students who volunteered. Select a primary and back-up notetaker.

5. If no one volunteers to take notes on the first or second day of class, notify your DAC immediately.

6. Select a primary and a secondary notetaker.

7. Refer the primary notetaker to the DRC Business Services Area, Matthews Center 143 to sign the Note-Taking Agreement Contract and receive further information within two weeks of signing the Note-Taking Agreement.

8. Provide note-taking guidelines to notetaker volunteers upon request.

9. Attend all classes.

Faculty Responsibilities:

1. Discuss the note-taking accommodations request with the student.


2. Clarify the procedures with the student’s designated Disability Access Consultant (DAC) or call the DRC Administrative Associate at (480/965–1234).

3. Read the in-class announcement, verbatim, to the class, and distribute note-taking packets to volunteers.

4. Collect packets from volunteers after class and give them to the student requesting note-taking accommodations.

5. Assist the student by reviewing notes for accuracy and recommend the most qualified primary notetaker when asked.

6. Read the in-class announcement at the beginning of the second class if no one volunteers in the first class.

7. Maintain student confidentiality.

Primary Notetaker Responsibilities:


1. Sign the Note Taking Agreement Contract at the DRC, Matthews Center Room 143.

Note: To receive full payment of the stipend or a letter of recommendation the Note Taking Agreement Contract must be signed within two weeks of the hire date. Payment or the letter is delivered approximately two weeks following the end of the semester.

2. Provide clear and concise notes to the student in accordance with the Note-Taking Agreement Contract. Copies can be made at Matthews Center, Room 143.

Note: The student requesting note-taking accommodations must attend class to receive notes. The only exception to this policy would occur if an interpreter for a deaf student fails to attend class. In this case, the deaf student may leave the classroom, but the notetaker should continue to take notes. When the notetaker is unable to attend class, he or she should notify the student and the DRC Administrative Associate or designee (480/965–1234) 24 hours in advance of the absence.

3. Supply copies of notes when requested by the student or faculty.

4. Notify the DRC Administrative Associate within two weeks if dropping the class, and submit a letter requesting payment for services completed.

DRC Administrative Associate Responsibilities:

Pay the stipend or provide a letter of recommendation to the volunteer based on the signed Note-Taking Agreement Contract at the end of each semester.
