University Libraries Forms and Procedures
Electronic versions of library forms are now available. In many cases, the forms are available in two versions: PDF format and Microsoft Word format.

PDF versions: You will need to have Adobe Acrobat Reader. If you don't have this software, you may download it for free from Adobe. You will need to print out to the form and then fill it out.

MS Word versions: If you have Microsoft Word, the form will automatically open in MS Word and you can fill it out electronically. If you have the MS Word Viewer for your web browser, you will need to print out the form and then fill it out.
