Proxy Card

 

What is it?

Library users may authorize another individual, or proxy, to use their ASU ID card for all library transactions.  These proxies may be Teaching Assistants, couriers, spouses, family members, etc.  Community card borrowers are not eligible for this service. 

 

How to apply

To apply for a proxy card, download and print the application form or pick a form up at a circulation desk at any campus library.  Enter in the information in the requested fields, and sign the form.  Turn in the completed form to the library's circulation desk.  The library will send a "proxy card" to you for your proxy to use when checking out materials in your name.

Conditions and agreements

It is important to remember the following conditions when requesting this service:

  • The named individual has your permission to use your ASU ID card to check out library materials in your name
  • You are responsible for all library materials checked out using your ASU ID card including their proper return to the libraries and any fines incurred.
  • At the time of checkout, proxies must present both their own ID card and your ASU ID or proxy card.

The status of proxies will be reviewed each May by library personnel.  All Teaching Assistants will have their proxy status expired in May.  Faculty should submit a new completed form to extend their Teaching Assistants' status.

 

You may give someone who is not a designated proxy permission to check out library materials with your ASU ID card.  To do so, please have them present their own ID, your ASU ID and a signed, dated, written permission from you.
