| EXCLUSIVE PROVIDER ORGANIZATION (EPO) An EPO is similar to a traditional HMO and provides the lowest-cost benefit because it uses contracted network physicians, hospitals, ancillary healthcare providers and facilities. Although it is recommended that everyone have a Primary Care Physician, it is no longer necessary to name one. You have access to any physician (including specialists) within your network without notifying the Plan Administrator. |

| PREFERRED PROVIDER ORGANIZATION (PPO) With a PPO you can use both in-network and out-of network tiers of physicians, hospitals and facilities. In-network services cost less out-of-pocket, but the out of-network tier offers more provider choices. |

| OUTSIDE ARIZONA An employee whose permanent residence is not in Arizona must enroll in Beech Street, a national PPO network. |

Effective February 1, 2008, Strategic Health Development Corporation will begin providing Medical Management services for members in the Schaller Anderson, Arizona Foundation, and RAN+AMN networks. Strategic will assist health plan members with medical education, nurse line services and care facilitation needs. Medical Management provides oversight of provider and treatment quality and effectiveness. * Education to help members and their families understand medical conditions and treatment plans * Coordination of discharge planning from the hospital * Assistance with making health care decisions * Review of potential risks and consequences of proposed medical services |
