How do I print a summary of my benefit elections?
It is simple.  

  • Log into my.asu.edu
  • Click on My Benefits Summary.
  • For future dated benefits, input the future Date and Click Go.

You should see a list of all of your benefits elections. If you do not, please contact HR immediately:

Why do I have to provide Social Security Numbers (SSN) for all dependents enrolled in my Benefit Options?
This is a federal requirement in accordance with the Mandatory Insurer Reporting Law (Section 111 of Public Law 110-173). The law took effect Jan. 1, 2009.
Follow these instructions to add your dependents’ Social Security numbers.

Why is there a difference in premium tier structure for Medical than Dental & Vision?
Yes, a four-tier option has been added to the medical plan only.  The Employee + 1 tier has been divided into Employee + Adult and Employee + Child.  This change more accurately reflects the true cost of dependents.
In addition, a three-tier option—Employee + One—was added to the Vision Plan this year.

Has there been any new legislation that will affect the benefits?
Yes. Here is a summary of the new legislation effective with the 10/01/09 – 09/30/2010 year.

  • Autism Coverage – Prohibits group health plans from denying coverage, imposing dollar limits or charging higher deductibles/copays based solely on the diagnosis of autism spectrum disorder.  It also requires plans to cover the cost of behavior therapy up to $50,000 per year for a child up to age 9 and $25,000 per year for a child age 9-16.
  • HEART Act – In accordance  with Heroes Earnings  Assistance and Relief Tax Act of 2008 (the HEART Act), qualified military reservists who participate in a flexible spending account (FSA) program may withdraw FSA funds (and avoid the use-it-or-lose-it rules) when they are called to active duty for 180 days or more, or for an indefinite period.  The withdrawal must be made during a period beginning on the day the reservist is called to active duty and ending on the last day of the coverage period of the FSA plan that occurs during the period of active duty.
  • Michelle’s Law – A new federal law allows continued coverage for seriously ill college students.  A college student will be able to maintain health plan eligibility for up to one year after full-time student status is lost due to a medically necessary leave of absence from school.
  • COBRA Premium Assistance – Under a new federal law, a person involuntarily terminated between Sept. 1, 2008, and February , 2010, may be eligible for COBRA premium assistance. Under this program, the individual pays 35% of the COBRA premium and the federal government subsidizes the remaining 65%.
  • Genetic Information Nondiscrimination Act (GINA) – Under a new federal law, group health are prohibited from adjusting premiums of contributions amounts for a group on the basis of genetic information.  A health plan also is prohibited from requiring an individual or his/her family member to undergo a genetic test, although the plan may request that a voluntary test be taken for research purposes.

How do I add or drop a dependent on my insurance?
Medical, dental and vision insurance is part of a pre-tax plan. Dropping or adding a dependent can be done:

  • During Open Enrollment (which occurs in the fall with an effective date of October 1)
  • During the plan year when a qualifying event occurs.

    Examples of qualifying events are:

  • Change in marital status:
    marriage, legal separation, divorce, annulment, or death of a spouse
  • Change in number of dependents:
    birth, adoption, placement for adoption, or death of a dependent
  • Change in dependent eligibility:
    dependent marries, a full-time student or older child reaches the age of 25, dependent goes on a mission
  • Change in employment status or work schedule of employee, spouse or dependent:
    new job, loss of job, change from full-time to part-time
  • Change in residence: that affects network availability

     Any change of election must be consistent with the qualifying event.
     IMPORTANT: You must notify the Benefits Office within 31 days of the qualifying event to be eligible to change your benefit elections. Print, complete and submit a Benefits Declaration For Change Form to the ESC.

IMPORTANT: You must notify the Benefits Office within 31 days of the qualifying event to be eligible to change your benefit elections. Print, complete and submit a Benefits Declaration For Change Form
to the ESC.
Any change of election must be consistent with the qualifying event (e.g. If an employee marries, the employee may add the new spouse or terminate coverage and become a dependent on the spouses's plan; but the marriage would not allow the employee to change networks within ASU's plan).
