E-FAX for Background Checks

You can now submit background and fingerprint check requests via our Electronic Fax Service (E-FAX),
an upgrade from our previous process that prevents transaction loss and make our site more secure.
Please send requests for background and fingerprint checks to one of the following options:
To send from a fax machine:
Enter 480.993.0006, this is not an ASU #, so you will need to use the procedure for an outside line, then send as usual - Faxes sent to this number will be delivered within five minutes of the completion of the fax transmission.
To send via email:
- Create a new email message
- In the TO BOX :
- Enter backgroundcheck@asu.edu.
- In the SUBJECT BOX:
- Type in either Background Request OR Fingerprint Request AND the applicant's name
- Attach file(s) you want to submit, including the HR Staffing Background Fax Sheet
- In lieu of the HR Staffing Background Fax Sheet, you can type in the following information in the body of the email
- agency org to charge
- HR person within your department authorized to receive results
- if there are any additional services requested (e.g. employment verification, MVD, etc.)
- Click Send
