1.THE
NUMBER OF REGULAR COUNSELING APPOINTMENTS are cumulative for each student during his or her career at ASU. When returning for services after termination, students will be charged based on the number of sessions used as reflected in the schedule above. Only Personal Consultation appointments are provided at no charge. Using Personal Consultations in lieu of regular appointments is not permitted. Any partial session due to late attendance on the part of the student will be charged as a full session.
2.COUPLES
COUNSELING: A couple is entitled to their first (3) regular couples appointments at the rate of $10 per session, regardless of any individual services one or both of the students have previously received. Charges for couples services are the joint responsibility of any ASU student(s) involved. Therefore, when a couple comprises two ASU students, any amount billed will be split and applied in equal parts to each student’s account.
3.NO SHOWS
OR LATE CANCELLATIONS for Personal Consultation appointments will be charged as a regular session. Late arrivals will be rescheduled and a “no show” fee charged.
4.INVOLUNTARY
TERMINATION OF SERVICES AND SUSPENSION OF ELIGIBILITY: After two consecutive no-shows or late cancellations, a student’s services at C&C may be terminated and the student supplied with a referral to services elsewhere. An excessive overall number of no-shows or cancellations may result in the suspension of eligibility for C&C services for up to one year from the last occurrence.
5.FEE
REDUCTIONS: Fees for C&C services may be reduced or waived upon demonstration of financial hardship. Fee reduction applications may be requested at the C&C front desk. Decisions about financial need are made in conjunction with an assessment of financial need provided by the Student Financial Assistance Office. Please Note: Fee reductions are not applied retroactively. A student is responsible for all charges for services received prior to being granted a fee reduction or waiver. Fee reductions and waivers do not apply to charges for no-shows and late cancellations. Students granted fee reductions or waivers will be charged full fee amounts for no-shows and late cancellations. If a fee reduction or waiver is granted, it will expire July 31 of the current year.
6.PAYMENT
FOR SERVICES: Payment for individual services is due at the beginning of each appointment; for group services, payment is due at the beginning of the second session. Students may (1) pay at Cashiering Services on the second floor of the Student Service Building and bring a receipt to the Front Desk or (2) have charges billed to their ASU student accounts on a monthly basis. Fees and policies are subject to change.
7.INSURANCE:
C&C does not serve as a provider for any insurance benefit, including Student Health Insurance.
