Optional Pre-Conference Workshops - Thursday, December 4th:
Pre-registration for these workshops is required, and space is limited. An additional fee is required for each workshop. Continental breakfast is included.

Pre-Conference workshops run concurrently from 9 a.m. to noon on the morning of Thursday, Dec. 4th.

Central Ave, Suite 500, Phoenix, AZ 85004-0691. A $25 processing fee will be charged against all refunds. Refunds will be made in the same payment form as original submission. THERE WILL BE NO REFUND FOR ANY CANCELLATION RECEIVED AFTER 5 P.M. THE DAY BEFORE THE CONFERENCE (DEC 3rd). THERE WILL BE NO REFUND FOR NO-SHOWS.
