Merit Point Log Form Current members: use this form to report your six service points and two meeting points each semester. You may either e-mail the fully completed form to the Community Service Chair or hand-deliver it to the GBPhi mailbox (#28) on the Third Floor of the Memorial Union (MU) Building at the ASU Tempe Campus.
Current Member Renewal Form Current members: please use this form to renew your membership each year, updating important contact information, name changes, and recording your payment of annual dues. In order to maintain membership in Gamma Beta Phi, you MUST renew your membership each year and pay the annual chapter dues of $15. As a current member, your help in keeping the organization going is a vital asset to its success!
New Member Application Form Effective immediately, any student can apply for membership to GBPhi so long as they download, print, fill out, and send in their application, along with a 1-page copy of their DARS which states their cumulative GPA, to the specified address.
