Consent Forms


Before a child under the age of 18 may participate in a research study, permission must be obtained from a parent or legal guardian. This form describes the tasks to be performed, the length of time it will take to complete the tasks, and any payment. [pdf]

Children are also given the opportunity to agree to participate in a research study. The study is described to the child verbally as well as the option to stop at any time. This form documents the child’s verbal response to three questions regarding their willingness to participate. [pdf]

A digital photo is taken of each child for use in the course of testing. This form allows us to use that picture during the child’s visit (Research Only). If a picture is suitable for use in a presentation or other publication, another form will be sent to the parent or guardian for additional approval (For
Publication). [pdf]

This form is optional and used to report the ethnic and racial distribution of our research subjects to the funding organizations. The parent or research subject is given the option of reporting the ethnic and racial information. This form is not necessary to participate in a research study. [pdf]
