• Actual passport, signed, valid at least 6 month beyond stay with blank visa pages
• One visa application form per person, completely filled-out and signed
• One recent passport-type photograph (color or black and white)
• Visa application fee paid only by money order or casher's check.
• Visa processing fee and your mailing address

• 6 months Single entry visa: $50.00
• 6 months Double entry visa: $75.00
• 6 months Multiple entry visa: $100.00
• 12 months Multiple entry visa: $150.00
