2008 Alumni Appreciation Award Nomination Form
The Alumni Appreciation Award is presented to a non alumnus of ASU who has rendered extraordinary and exemplary service to Arizona State University, ASU Alumni Association and/or the community. The Alumni Service Award is presented during half-time ceremonies of the Homecoming football game, November 15, 2008.

SELECTION CRITERIA:

In order to qualify for this award, a nominee must have:
  • Not attended ASU
  • Provided significant service to one or more of the following areas:
    1. Arizona State University
    2. ASU Alumni Association
    3. Community
     

SUPPORTING DOCUMENTATION:

All applicants must have the following in order to be considered for the award:
  • Listing of accomplishments, achievements, volunteerism and work history
  • Three (3) letters of recommendation
