SSM 901–02 Table

Request for a Faculty Associate Card

An individual requesting a faculty associate card should do the following:

  1. Have the sponsoring religious organization provide a written request containing the following information:

    1. the individual’s name, Social Security or ASU ID number, contact address, and telephone number
    2. a statement of the proposed nature of the ministry
    3. verification that the individual’s ministry at ASU is full-time

      and

    4. the specific duties performed by the individual for the ASU campus community.
  1. Submit the request to the appropriate campus department director or designee.

The university department approves the request and send a memo to Human Resources, Sun Card Office, Computer Accounts Office, Sun Devil Fitness, and University Libraries verifying the individual is a faculty associate.

The individual requesting a faculty associate card goes to the Sun Card Office to get the faculty associate card.

The university department adds the faculty associate’s name to Human Resource’s Faculty Associate list.

Renewal of Faculty Associate Card

An individual desiring renewal of a faculty associate card provides the affiliated department with a written memo from the sponsoring religious organization requesting a renewal of the card and indicating the three individuals who will hold cards for the current year.

The university department notifies Human Resources, the Sun Card Office, the Computer Accounts Office, Sun Devil Fitness, and University Libraries of the individual’s Continuing Faculty Associate status.
