SSM 204–08: Declaration of Graduation

Purpose

To establish a Declaration of Graduation for undergraduate students relating to the student’s specific degree requirements for bachelor degree completion and graduation

Background

A system called DARS Web creates a degree audit, which is an automated record that matches courses a student has completed with the requirements of a particular academic degree program. Eligible students file a Declaration of Graduation in place of the Undergraduate Program of Study.

Policy

Undergraduate students following the curriculum requirements of the 1996–1998 or later General Catalog must file Declaration of Graduation forms no later than the semester in which they earn 87 semester hours for all campuses. Student-athletes must file a Declaration of Graduation during their first semester at ASU. A student who does not file a Declaration of Graduation on time is prevented from further registration.

Undergraduate students following the curriculum requirements of the 1994–1996 or earlier catalog must file an undergraduate program of study. See your academic advisor for information.

ProcedureD

Responsibility	Action
Undergraduate student	Meet with an academic advisor.
Advisor	Process degree audit and review with student.
Student	Obtain signatures of approval on the Declaration of Graduation form from the advisor and the dean’s representative. Give the completed, signed Declaration of Graduation form to the college.
College	Forward the approved Declaration of Graduation to the Graduation Section at the Tempe campus or, if the student is registered at the West campus, Registration Services at the West campus.
Graduation Section at the Tempe campus, Registration Services at the Polytechnic campus, or Enrollment Services at the West or Downtown Phoenix campus	Update OASIS to show Declaration of Graduation has been approved.
