The student obtains a Leave of Absence Request.
(If the LOA request is for two semesters or less and the student is not in the U.S. on an F1 or J1 visa, the student skips the next two steps.)
The college reviews the LOA request. If the student meets requirements, staff signs the form and returns it to the student. If the student does not meet requirements, staff does not sign the LOA request and notifies the student of ineligibility.
The student returns the completed application to the Recording/Readmission section (Tempe campus) or Enrollment Services (Downtown Phoenix, Polytechnic, and West campuses).
Recording/Readmission section (Tempe campus) or Enrollment Services (Downtown Phoenix, Polytechnic, and West campuses) verifies that the LOA request is complete and that the student meets LOA requirements. If the student does not meet requirements, the staff notifies the student in writing that the LOA has been disapproved.
If the student meets requirements, staff processes the LOA in OASIS and notifies the student and the student’s college in writing that the LOA has been approved.
The Recording/Readmission section (Tempe campus) or Admission Services at the West campus notify the student in writing of LOA expiration prior to the student’s return to ASU.
If the student requires additional leave, the student submits a new LOA request, following the procedures in this policy. If the total requested LOA duration is greater than a year, the additional time will not be granted without college approval.
If additional leave is not necessary, the readmitted student completes any outstanding items (e.g., submits transcripts from institution attended during LOA, meets with advisor, completes any to-do items, and clears any holds).
