Lost or Stolen Transit Passes, Permits, or Gate Access
Cards
The permit/transit pass holder:
- reports a lost or stolen transit pass, permit, or gate access card in person to
any campus Permit Sales Office, Parking and Transit Services
- completes a Parking Permit Application for a replacement
permit
- reads, completes, and signs the Lost/Stolen Permit
Affidavit
and
- presents an ASU ID card (or driver’s license) and pays the
replacement fee(s) for the permit or gate access card if applicable.
If the permit/transit pass holder finds the missing permit or gate access card, do not use it but return it to the Permit Sales Office.
The Permit Sales Office:
- verifies the identity of the person requesting the new permit or
gate access card and the requestor’s current ownership of
the permit or gate access card in question
- accepts replacement fee(s) for the lost or stolen transit pass, permit, or gate
access card
- enters the transaction into the parking management
database
- deactivates the lost or stolen permit or gate access card
- places lost or stolen permit numbers on the appropriate list for
enforcement officers
and
- reports lost or stolen transit passes to the Commuter Options office.
Damaged Permit or Gate Access
Card
The permit holder:
- if possible, turns in to the Permit Sales Office identifiable remnants of the damaged parking
permit (i.e., at least two of the permit numbers) or the gate access
card
- completes a Parking Permit Application for a new permit
and
- presents an ASU ID card (or driver’s license) and pays the
replacement fee(s) for the permit or gate access card.
The Permit Sales Office:
- verifies the identity of the person requesting the new permit or
gate access card and the requestor’s current ownership of
the permit or gate access card in question
- accepts the replacement fee and issues the replacement permit or
gate access card
- enters the transaction into the parking management database
and
- deactivates the damaged permit or gate access card.
