ASU assumes no responsibility for equipment on loan or demonstration unless the user has submitted an Equipment on Loan or Demonstration form to Purchasing and Business Services.
| Note: | If the form is not submitted, the Arizona Department of Administration’s Risk Management Division may not insure the equipment. |

The department prepares an Equipment on Loan or Demonstration form before accepting the equipment and sends a photocopy of the form to Purchasing and Business Services.
Purchasing and Business Services sends a photocopy of the form to Insurance Services.
Property Control tags the equipment and adds it to the Property Control database at zero dollars.
