Approval Signatures Required on Request for Computer Access Form
To add an Advantage ID, complete Sections 1, 3b, 3e, and 3g, if applicable. Approval signatures are required from the provost/vice provost/vice president, dean, department chair, director, or business operations manager for the area.
To change personal information only (e.g., title, location, telephone, mail code), complete Section 1 and write “CHANGE” at the top of the form. Only the employee’s signature is required.
Note: The mail code and college/VP area must be correct to ensure proper delivery of Advantage reports.
To change the department/college or VP area, complete Sections 1 and 3e and write “CHANGE” at the top of the form. Approval signatures are required from the provost/vice provost/vice president, dean, department chair, director, or business operations manager for the area.
To change security profile(s), complete Sections 1 and 3e and write “CHANGE” at the top of the form. Approval signatures are required from the provost/vice provost/vice president, dean, department chair, director, or business operations manager for the area.
