FAC 112 Table

The prospective donor contacts the assistant director, Grounds Services, at 480/965–7284.

If necessary, Facilities Management mails the appropriate form to the requester.

The prospective donor completes the form and returns it with the donation.

Facilities Management:

1. sends a copy of the gift or donation form to the ASU Foundation
2. purchases the tree and/or plaque
3. plants the donated tree and/or manufactures and installs the plaque

   and

4. holds a tree planting ceremony with the donor or mails a certificate to the donor for display and notifies the donor of the tree and/or plaque location.

The ASU Foundation mails a letter of appreciation and a tax credit receipt to the donor.
