EHS 402 Table

The individual experiencing discomfort or illness:

  1. informs his or her supervisor

    and

  2. provides his or her name, phone number, office or cubicle number, symptoms, possible sources of contamination, and any other information that may be relevant to the indoor air quality investigation.

The department:

  1. contacts Environmental Health & Safety at 480/965–1823 to file the complaint and request an indoor air quality investigation

    and

  2. provides Environmental Health & Safety with the relevant information.

The individual experiencing discomfort or illness completes and submits to his or her supervisor an Indoor Air Quality Questionnaire.

Environmental Health & Safety:

  1. contacts the supervisor and affected employees to confirm information regarding the complaint
  2. conducts occupant interviews and an indoor air quality investigation as needed
  3. determines mitigation measures and communicates the results, in writing, to affected employees and their supervisors

    and

  4. works directly with Facilities Management if the indoor air quality problem is attributed to the facility’s heating, ventilation, and air-conditioning system or another building system or condition.
