EHS 102: Confined Space Entry

Purpose

To protect employees from the hazards associated with entering and conducting operations in confined spaces

Arizona State University Confined Space Entry Plan

Applicability

Employees who, while performing their duties, enter a confined space

Policy

Departments and units shall implement the practices and procedures outlined in the Arizona State University Confined Space Entry Plan, a guidance document found on the Environmental Health & Safety Web site.

The Confined Space Pre-Entry Checklist and the Confined Space Entry Permit available from the Environmental Health & Safety forms Web page shall be completed prior to entering a confined space. Confined spaces shall be considered “permit-required” until the information obtained from the Confined Space Pre-Entry Checklist demonstrates otherwise.

Responsibility	Action
Department	Ensure that entry, standby, and backup employees are properly trained and authorized for their designated functions. Prior to performing any work in a confined space, an evaluation of the space must be performed to determine whether a permit is required.
Environmental Health & Safety	Provide assistance with technical and monitoring aspects of the confined space entry standard.
Employees	Conduct confined space operations in accordance with the Arizona State University Confined Space Entry Plan, including the Pre-Entry Checklist and the Confined Space Entry Permit procedures.
