Admissions Information

 

 

Visas:
In order to obtain a  Visa, we will require you to first be admitted into your prospective program. Your sponsor will be USAID and it is important you remain in contact with your graduate advisor from your department, ASU-USAID contact, and the graduate college at all times.

Medical Examination Form
Conditions for Training Form

In addition to these documents we will need a GOOD PHOTOCOPY of your Passport. Be sure it is in good condition.

Once you have received your DS-2019 Forms.

You will need to set up an appointment with your local consulate. BE SURE TO GET YOUR $100 FEE WAIVER FROM USAID PRIOR TO YOUR APPOINTMENT!
